The health board report for people with disabilities is a document issued by the delegations authorized by the Ministry of Health, which evaluates people's disability status, disability groups, types and degrees of disability, and is the basic document for benefiting from disability rights.

Question 6 : What are the special requirement levels specified by COZGER for children with disabilities to benefit from care in public and private institutions and for their relatives to receive home care assistance?

For children with special needs to benefit from care services, one of the phrases "Has a Very Severe Special Requirement", "Has a Prominent Special Requirement" or "Special Condition Requirement" must be written in the COZGER report.

Question 7 : How to object to the COZGER report?

The COZGER report can be appealed to the Provincial Health Directorate of the province where the child is taken or residing by the person who takes care of the child or the institution requesting the report, within thirty days (30) from the date the report is received and/or appears via e-government. The Provincial Health Directorate directs the child to a different hospital. The second COZGER report received as a result of the objection can be accepted and used without going to the third hospital if the person who takes care of it or the serving organization finds it appropriate. The Provincial Health Directorate directs the child to the referee hospital if the objection to the result of the second COZGER report continues.

Question 8: Can the COZGER report be renewed in another way other than objection?

If the special requirement of the child changes, the COZGER report can be renewed by referring it to the health board with the request of the person undertaking the care and the approval of the relevant branch physician.

The process of receiving reports is initiated by applying to hospitals authorized to give ESKR by the disabled person or his guardian.

Question 10: How is the objection to disability health board reports made?

Health board reports for people with disabilities can be appealed within thirty days (30) from the date the report is received and/or appears via e-government to the Provincial Health Directorate, which is resident or received by the disabled person, the guardian of the disabled person or the institution requesting the report.

Question 11: Can disability health board reports be renewed except for objection?

If a new disability situation arises or there is a change in the current disability, the ESKR renewal process can be initiated if the individual's request and the relevant branch physician considers the referral to the health board appropriate.

Question 12: What should be included in the ESKR for people with disabilities to benefit from care in public and private institutions and for their relatives to receive home care assistance? :

People with disabilities must receive a disability health board report to receive services from official care centers. For people with disabilities to receive services from private care centers, the health board report must have the phrase "fully dependent" or "severely disabled" (for reports obtained under the old legislation) marked.

Question 13: How to get an ID card for people with disabilities?

To obtain a Disability ID Card, the disabled person must document that he/she is 40% disabled with a health board report. Mild ÖGV (40-49,) Moderate ÖGV (50-59), Severe ÖGV following the provisions of the "Regulation on Special Needs Assessment for Children" in the health board report if the disabled person is under 18 years of age (60-69) Very Severe ÖGV (70-79), Has a Significant Special Requirement (80-89), Has Special Condition Requirement (90-99) must be included.

The Provincial Directorate of Family, Labor and Social Services in the province where the disabled person resides can be applied with 2 photos, an identity card, and an original or approved sample of the disability health board report and an ID card can be obtained for the disabled.
